Guinea Pig Hypovitaminosis C (Scurvy): Recognition and Treatment

Hypovitaminosis C, or scurvy, is one of the most common and most preventable diseases in the pet guinea pig (Cavia porcellus) — and one of the most frequently missed. Guinea pigs share with primates, some bats, and a handful of other species the loss of a functional enzyme in the vitamin C synthesis pathway, which makes them wholly dependent on dietary ascorbic acid. Because the early signs are vague (a quiet, unthrifty animal that eats less and moves less) and because deficiency often coexists with the very husbandry problems that caused it — poor-quality diet, dental disease, secondary infection — scurvy should sit near the top of the differential list for almost any ill or "failing to thrive" guinea pig. Recognising it early, correcting the deficiency, and fixing the underlying diet reliably turns around a patient that might otherwise be worked up for weeks.

Why guinea pigs are obligate dietary-C dependent
Most mammals synthesise ascorbic acid endogenously in the liver from glucose. The final step of that pathway is catalysed by the enzyme L-gulonolactone oxidase (GULO). Guinea pigs carry a non-functional, mutated GULO gene and produce no active enzyme, exactly as humans do. They therefore cannot make any vitamin C at all and must obtain their entire daily requirement from the diet.
Vitamin C is an essential cofactor for prolyl and lysyl hydroxylase, the enzymes that hydroxylate proline and lysine residues during collagen synthesis. Without adequate ascorbate, collagen is under-hydroxylated, structurally weak, and poorly cross-linked. The clinical consequences follow directly from failure of collagen-dependent tissues: fragile capillaries and periosteum (haemorrhage), poorly mineralised and weakened growth plates (bone and joint pain), defective dentine and periodontal ligament (dental disease), impaired wound healing, and compromised immune function with increased susceptibility to secondary infection.

Body stores are small, so clinical signs can appear within a few weeks of inadequate intake.
Aetiology and risk factors
Scurvy in pet guinea pigs is almost always a husbandry failure rather than a primary metabolic disease. Contributing factors include:
- Inappropriate diet — feeding rabbit pellets (which are not supplemented with vitamin C), muesli-style mixes with selective feeding, or a diet dominated by grass hay without a stabilised C source or fresh vitamin-C-rich vegetables.
- Degraded pelleted feed — even guinea-pig-specific pellets are supplemented with ascorbic acid that oxidises steadily once the bag is opened and exposed to light, heat, air, and humidity. Much of the added C may be gone within roughly 90 days of milling, so "medicated" pellets are not a guarantee of adequate intake.
- Vitamin C in the water — ascorbic acid added to drinking water degrades quickly, alters taste, and can reduce total water intake; it is an unreliable delivery method and a common reason owners believe they are supplementing when they are not.
- Reduced intake from any cause — dental disease, pain, or concurrent illness that lowers food consumption will precipitate deficiency in an animal with no reserve.
Clinical signs
Signs range from vague to dramatic and frequently overlap with the primary problem. Common presentations:
- Unthriftiness and poor coat — weight loss, a rough or unkempt hair coat, reduced grooming, and general lethargy or depression.
- Lameness and reluctance to move — often the most striking sign. Subperiosteal and epiphyseal haemorrhage, along with weakened growth plates, produce painful bones and joints. Affected guinea pigs are reluctant to walk, may adopt a hunched posture, resent handling, and sometimes cry out when lifted. Swollen, painful joints and long bones are typical.
- Dental disease — defective dentine and weakened periodontal attachment contribute to malocclusion, loose or overgrown teeth, and secondary anorexia and ptyalism.
- Haemorrhage — gingival bleeding, cutaneous or mucosal petechiae and ecchymoses, and bleeding into muscle and joints reflect capillary fragility.
- Poor wound healing and immune compromise — chronic non-healing wounds, and a marked predisposition to secondary bacterial infection. Concurrent bacterial pneumonia, pododermatitis, cystitis, conjunctivitis, or enteric disease is common, and a septic or pneumonic guinea pig should always prompt evaluation of vitamin C status.
- Diarrhoea and reduced appetite — nonspecific but frequent.
Diagnostic workup
Diagnosis is primarily clinical and rests on a careful dietary and husbandry history combined with a compatible clinical picture. Ask specifically what pellet is fed, how old the open bag is and how it is stored, what fresh vegetables are offered and how often, and whether vitamin C is being added to water (and if so, how it is prepared).
- History and physical examination — the diagnostic cornerstone. A guinea pig on rabbit pellets or an old bag of feed, presenting with lameness, poor coat, and reluctance to move, is scorbutic until proven otherwise.
- Radiographs — useful supportive imaging. Look for widened, irregular costochondral junctions, epiphyseal and metaphyseal changes, subperiosteal reaction from haemorrhage, and generalised reduced bone density. Radiographs also help evaluate concurrent dental disease and rule out fracture or other causes of lameness.
- Oral examination — sedated or conscious skull/oral exam to assess dental disease, which is both a cause and a consequence of reduced intake.
- CBC and biochemistry — nonspecific but valuable for identifying secondary infection, dehydration, and organ involvement, and for guiding supportive care.
- Plasma or leukocyte ascorbate assay — quantitative confirmation is possible but rarely necessary or practical, and treatment should never be delayed to obtain it.
A pragmatic and widely accepted approach is the response to supplementation: clinical improvement following vitamin C repletion strongly supports the diagnosis.
Treatment
Treatment has two parts — repleting vitamin C and addressing the diet and any secondary disease. Do not wait for confirmatory testing; begin supplementation on clinical suspicion.
Therapeutic vitamin C supplementation. For clinically scorbutic patients, give oral ascorbic acid at a therapeutic supplementation dose — higher than maintenance — continued until clinical signs resolve and the animal is reliably eating a properly supplemented diet, after which intake is tapered to the daily maintenance requirement once recovered. Injectable ascorbic acid may be used parenterally in anorexic or critically ill patients that will not take oral medication, transitioning to oral dosing once feeding resumes. Vitamin C is water-soluble and has a wide safety margin, so short-term therapeutic dosing is well tolerated.
Supportive and secondary-disease care. Scorbutic guinea pigs are frequently anorexic, in pain, and often infected. Management commonly includes assisted syringe feeding with a herbivore critical-care formula, fluid therapy for dehydration, and analgesia. Because secondary bacterial infection is common, targeted antibiotic therapy is often required — always with attention to the guinea pig's sensitivity to antibiotic-associated enterotoxaemia, so agents that disrupt the caecal flora — beta-lactams (penicillins, aminopenicillins, and cephalosporins) by any route, plus clindamycin, lincomycin, erythromycin, and related narrow-spectrum agents — must be avoided. Monitoring and prognosis
Most uncomplicated cases respond quickly, with improved demeanour, appetite, and mobility within several days of starting supplementation and dietary correction. Continue therapeutic dosing until signs resolve, then confirm the owner can sustain adequate maintenance intake. Recheck body weight, appetite, and mobility, and reassess dental disease and any secondary infection at follow-up.
The prognosis is good with early recognition, prompt supplementation, and a corrected diet. It worsens with advanced disease, severe secondary infection or sepsis, significant dental disease, or when the underlying husbandry problem is not addressed and the animal relapses.
Prevention and client education
Prevention is straightforward and is the single most valuable outcome of the consultation:
- Feed a guinea-pig-specific, vitamin-C-stabilised pelleted diet, never rabbit pellets, and buy pellets in quantities used within about 90 days. Store feed sealed, cool, dark, and dry, and discard old bags — supplemented C degrades whether or not the bag is opened.
- Offer fresh vitamin-C-rich vegetables daily (for example bell pepper, leafy greens, and other C-rich produce), introduced gradually and appropriate to the individual.
- Provide unlimited grass hay as the dietary foundation for dental and gastrointestinal health.
- Avoid relying on vitamin C in the drinking water — it degrades rapidly, changes taste, may reduce water intake, and gives false reassurance. Direct oral supplementation is preferable when supplementation is needed.
- Increase intake during pregnancy, growth, illness, and stress, when requirements rise.
Emphasise to owners that guinea pigs cannot store or make vitamin C, so supplementation is a daily, lifelong commitment.
When to refer
Consider referral to a veterinarian experienced in exotic companion mammals when significant dental disease requires advanced imaging (CT) or specialist dental treatment, when there is severe secondary infection or sepsis requiring intensive supportive care, when the patient fails to respond as expected to appropriate supplementation and dietary correction (prompting reconsideration of the diagnosis), or when complex concurrent disease exceeds the resources available in-house.
