Guinea Pig Ovarian Cysts: Diagnosis and Treatment

Cystic ovarian disease is so common in the intact female guinea pig that it is better regarded as an expected feature of ageing than as an uncommon diagnosis. Prevalence rises steeply after eighteen months to two years of age, and by middle age a large proportion of unspayed sows carry ovarian cysts of some type, many of them clinically silent for a time before declaring themselves with alopecia, an abdominal mass, or vague signs of pain and inappetence. Because the condition is both prevalent and eminently manageable — often curable, with definitive surgery — it deserves a place near the top of the differential list for almost any middle-aged intact sow that is losing hair, off her food, or developing coelomic distension.

Two kinds of cyst
Not all guinea-pig ovarian cysts are the same, and the distinction matters clinically:
- Rete ovarii (serous) cysts are the most common type. They arise from the rete ovarii and are generally non-hormonally-active, thin-walled, fluid-filled structures. They can grow large enough to cause a palpable mass, abdominal distension, pain, and pressure on adjacent organs, but they do not typically drive the endocrine signs.
- Functional (hormonally active) cysts, including follicular cysts, produce oestrogen (and in some cases other hormones) and are responsible for the classic endocrine presentation — bilaterally symmetrical flank alopecia and behavioural change. These are frequently associated with cystic endometrial hyperplasia and other uterine pathology.
A given sow may have cysts of more than one type, and the type influences whether hormonal signs are present and how well medical management is likely to work.
Clinical signs
The presentation spans an endocrine picture, a space-occupying picture, or both:
- Bilaterally symmetrical, non-pruritic flank/dorsal alopecia — the hallmark of hormonally active cysts, typically sparing the head and limbs and without the inflammation of a primary skin disease
- Palpable abdominal mass(es), often bilateral, and progressive abdominal distension
- Reduced appetite, weight loss, lethargy, and signs of abdominal pain (hunched posture, reluctance to move) as cysts enlarge
- Behavioural changes — nesting, mounting, or aggression — attributable to hormonal activity
- Occasionally decreased fertility in breeding sows, and signs referable to concurrent uterine disease such as haemorrhagic vaginal discharge
- Many cysts are incidental findings on palpation or imaging in an otherwise well animal, which is itself a management decision point
Diagnosis
- Abdominal palpation frequently detects one or both enlarged, fluid-filled ovaries as smooth, rounded, sometimes tense masses dorsal in the caudal abdomen; large cysts are readily appreciated, small ones easily missed.
- Abdominal ultrasound is the key diagnostic tool. It characterises the number, size, wall thickness, and internal character of the cysts (simple anechoic fluid versus complex or septate contents), helps distinguish ovarian cysts from other abdominal masses and from uterine pathology, and guides decisions about drainage versus surgery. It is the single most useful test in the workup.
- Radiographs demonstrate a space-occupying soft-tissue mass and coelomic distension and help rule out other causes, but define the lesion far less well than ultrasound.
- Assess for concurrent uterine disease (cystic endometrial hyperplasia, pyometra, neoplasia), which commonly accompanies ovarian cysts and influences the choice of treatment.
Treatment
Ovariohysterectomy — the definitive option
Ovariohysterectomy (OVH) is the definitive treatment and is preferred in most cases, particularly where cysts are large or recurrent, where hormonal signs are marked, or where concurrent uterine pathology is present. Removing both ovaries and the uterus resolves the current disease and prevents recurrence and future uterine problems. The trade-off is a coelomic surgery in a species with meaningful anaesthetic and peri-operative risk, so patient stabilisation and a careful surgical and supportive plan are essential (see below).
Hormonal management
For the hormonally active cyst — or as a temporising or non-surgical option in a poor surgical candidate — hormonal therapy can shrink functional cysts and resolve the associated alopecia and behavioural signs. GnRH agonists such as deslorelin or leuprolide, and hCG, are described for this purpose. Responses are most reliable for hormonally active cysts and are often temporary, with recurrence common; hormonal management does not address rete cysts or concurrent uterine disease, and is best viewed as a bridge or an alternative for animals that cannot undergo surgery rather than a substitute for OVH in a good candidate.
Ultrasound-guided drainage
Percutaneous, ultrasound-guided cyst drainage relieves the pressure and pain of a large fluid-filled cyst and can be useful palliation or a stabilising step before surgery, but recurrence is high because the cyst wall remains, and repeated drainage carries its own risks. It is a palliative or temporising measure, not a cure.
Peri-operative considerations specific to guinea pigs
Guinea pigs are demanding surgical and anaesthetic patients, and the peri-operative plan is as important as the procedure:
- Antibiotic selection with dysbiosis in mind. Guinea pigs are exquisitely sensitive to antibiotic-associated enterotoxaemia; narrow-spectrum, Gram-positive-targeting antibiotics (notably many beta-lactams) can be fatal by disrupting the caecal flora. If an antimicrobial is indicated, it must be chosen from agents considered safe in this species — confirm on the guinea pig formulary.
- Analgesia with an NSAID such as meloxicam and, for surgical pain, an opioid such as buprenorphine, dosed from the source-cited figures.
- Assisted feeding and gut motility. Guinea pigs must keep eating; provide critical-care formula by syringe peri-operatively and prokinetic support where indicated to prevent post-operative ileus.
- Vitamin C support. Guinea pigs cannot synthesise vitamin C, and the stress of illness and surgery raises demand; ensure adequate supplementation peri-operatively.
- Thermal support and careful fluid management in a small patient prone to hypothermia and perfusion problems under anaesthesia.

Prevention
The most effective prevention is ovariohysterectomy of young sows before cystic disease and uterine pathology develop. Where routine juvenile spaying is not elected, owners of intact sows should be counselled that cystic ovarian disease is highly likely with age and advised to watch for symmetrical hair loss, abdominal enlargement, and appetite change, prompting earlier assessment.
Common mistakes
- Attributing symmetrical flank alopecia to a primary skin disease and pursuing dermatological workups while missing the ovarian cause.
- Relying on radiographs alone and under-characterising the lesion when ultrasound would define it.
- Treating hormonal cysts medically and forgetting concurrent uterine disease, which hormonal therapy does not resolve.
- Repeated cyst drainage as a definitive plan, accepting high recurrence rather than offering OVH.
- Neglecting guinea-pig-specific peri-operative care — unsafe antibiotic choice, inadequate analgesia, no assisted feeding or vitamin C — turning a routine spay into an enterotoxaemia or ileus fatality.
When to refer
Consider referral to an exotic-mammal surgeon when cysts are very large or vascular, when concurrent uterine disease complicates the surgery, when the patient is a poor anaesthetic candidate, or when the practice is not equipped for guinea-pig-appropriate anaesthesia and post-operative support.
