Long-acting irreversible cholinesterase inhibitor. Produces intense, prolonged miosis and sustained reduction in intraocular pressure by enhancing trabecular outflow.
Rare — limited availability. Very potent miotic. Systemic cholinergic effects possible. Use for refractory glaucoma when other agents fail. Monitor for GI and respiratory signs.
