Posaconazole is dosed at 5–7.5 mg/kg PO q24h (or divided into 2 doses/day) in cats, per Plumb's Veterinary Drug Handbook, 9th ed., Posaconazole, Dosages, corpus p.1071 (printed p.1044), CATS row b. Verify against current literature before clinical use.
Inhibits lanosterol 14-alpha demethylase (CYP51). Broadest spectrum triazole with activity against Aspergillus, Candida, Zygomycetes, and others.
Oral suspension requires fatty food for absorption. Tablet form has better bioavailability. QT prolongation risk. Hepatotoxicity monitoring recommended. Limited veterinary data.
