2-PAM, Protopam
Reactivates acetylcholinesterase by cleaving organophosphate from enzyme active site. Must be given before aging occurs (irreversible binding).
Give within 24-48 hours of organophosphate exposure (before aging). Always co-administer atropine. Not effective for carbamate poisoning (unnecessary). Slow IV push.
